The Community and Geriatric Evaluation and Management (GEM) is a multidisciplinary service that provides short term in home assessment, brief intervention and support for the older person with complex health needs.

The service assists older people who:

  • Have had recent multiple admissions to hospital or the Emergency Department
  • Have health issues that require a short term multidisciplinary assessment and case management
  • Are experiencing a recent decline in health, cognition (especially memory) or ability to self-care which increases the risk of being admitted to hospital
  • Have health conditions that can be assessed or managed safely in the home environment
  • Are willing to participate actively in a program to address health issues and take on strategies to maximise independence and the ability to safely remain in the home
  • Would benefit from establishing services and supports to manage health in the community


  • Comprehensive geriatric assessment
  • Short term interventions and case management
  • Support for older people with complex health needs to establish a longer term support network to address health concerns
